All parents of student-athletes must complete the ONLINE REGISTRATION process for each sport played. The online registration replaces all hard copies of forms (however, the physical form may be uploaded during the online registration process OR be brought into the Athletics and Activities Office). In addition to registering online, the following requirements must also be met and verified by the Athletics Office. Once this is done, the student-athlete will have BLUE status. If a student is able to practice, but NOT eligible for competition, a SILVER status will be issued.
Please note: we no longer hand out actual blue cards. An athlete's status will be considered "blue" once all requirements are met and coaches will be made aware of each registrant's status. IF A STUDENT-ATHLETE'S NAME IS NOT ON THE COACH'S LIST, NO PARTICIPATION OR PHYSICAL ACTIVITY OF ANY KIND WILL BE ALLOWED!

A physical must be on file for all student-athletes. Most students complete the physical form during their freshmen and junior years. Please reference the table below for valid dates in regard to all physicals. Our new online system allows families to either upload their physical during the registration process or turn in their hard copy when ready.

Student-athletes must have a minimum 2.0 GPA with no more than one F/I. When quarter and semester grades are available at a grade checkpoint, the more favorable grades for the student will be used to determine eligibility. Incoming freshmen for the 2017-2018 school year will not have their grades officially checked until the end of 1st quarter.
Student-athletes must pay a $150 athletic fee in order to participate in athletics at any level. There is no sport limit, and no family cap. If the student-athlete does not make the team, or chooses to leave the team prior to the first competition date, a full refund may be requested. All fees are to be paid through Rev Trak.
Student-athletes should have no outstanding financial obligations owed to the school district at the time of registration.
All incoming freshmen, new/transfer students, as well as any student-athlete who did not participate in athletics at Waukesha West High School during the previous school year must attend our Freshmen/New Student Orientation Meeting, typically held in August. During this meeting we'll discuss the Code of Conduct, WIAA Eligibility, and many other relevant topics.
Student-athletes and parent(s)/guardian(s) must complete the state-mandated Concussion Sign-off, included within the online registration.
Student-athletes and parent(s)/guardian(s) must complete the School District of Waukesha Spectator Code of Conduct, included within the online registration.
Student-athletes and parent(s)/guardian(s) must complete a Health Information Form and make any necessary updates each sports season within Infinite Campus. This is included within the online registration.
