Athletic Eligibility Requirements at Waukesha West High School

All student-athletes must complete each of the following requirements listed below in order to compete for Waukesha West.  Once all requirements have been received and verified by the Athletic Office, the student will be issued a BLUE CARD.  If a student is able to practice, but NOT eligible for competition, a SILVER CARD will be issued.  The card should then be presented to the coach on the first day of practice.  No paperwork or fees should ever be given to a coach!  The Athletic Office is open daily 7am-3pm.


In order to register for a sport, simply visit the link below and follow the prompts.  If your child participated in a sport earlier this school year, you need to fill out the first few prompts in order for him or her to be registered and to receive a new blue card. Blue cards do not transfer from sport to sport.



If your student participated in a fall or winter sport, complete this form as directed and have your student see Mrs. Dutelle for his/her BLUE card.

If this spring is your student's first sport season of the school year, complete this form as directed, pay the athletic fee online and submit your student's physical form (or confirm a valid physical is on file) to Mrs. Dutelle. Your student will then be eligible to receive a BLUE card.

Step #1

A physical must be on file for all student-athletes.  Most students complete the physical form during their freshmen and junior years, while completing the alternate year form sophomore and senior years.  Please reference the table below for valid dates in regard to all physicals.

Step #2

Student-athletes must have a minimum 2.0 GPA with no more than one F/I.  When quarter and semester grades are available at a grade checkpoint, the more favorable grades for the student will be used to determine eligibility.  Incoming freshmen for the 2017-2018 school year will not have their grades officially checked until the end of 1st quarter.

Step #3

Student-athletes must pay a $150 athletic fee in order to participate in athletics at any level.  There is no sport limit, and no family cap.  If the student-athlete does not make the team, or chooses to leave the team prior to the first competition date, a full refund may be requested.  All fees are to be paid through Rev Trak.

Step #4

Student-athletes should have no outstanding financial obligations owed to the school district at the time of registration.

Step #5

All incoming freshmen, new/transfer students, as well as any student-athlete who did not participate in athletics at Waukesha West High School during the previous school year must attend our Freshmen/New Student Orientation Meeting, typically held in August.  The date of 2018's meeting is TBD and will be announced on the website once determined. During this meeting we'll discuss the need to sign the Code of Conduct and WIAA Eligibility, among many other relevant topics.


Step #6

Student-athletes and parent(s)/guardian(s) must complete the state-mandated Concussion Form once per school year.  This step is part of the online registration (see link above).

Step #7

Student-athletes and parent(s)/guardian(s) must complete the School District of Waukesha Spectator Code of Conduct once per school year.  This step is completed as part of the online registration process using the link above.

Step #8

Student-athletes and parent(s)/guardian(s) must complete a Health Information Form every year and make any necessary updates each sports season or acknowledge there are no changes. This step is completed as part of the online registration process using the link above.
