Forms & Registration

Athletic Eligibility Requirements at Waukesha West High School

All student-athletes must complete ONLINE REGISTRATION which includes each of the following requirements listed below in order to compete for Waukesha West.  Once all requirements have been received and verified by the Athletic Office, the student will have BLUE status. If a student is able to practice, but NOT eligible for competition, a SILVER status will be issued.

NEW FOR 2018, WE WILL NOT USE ACTUAL CARDS THAT ARE PICKED UP IN THE OFFICE.  You may call or stop in to verify your registration is complete, and the head coach of your sport will have a listing of all athletes who have completed the process.  IF YOUR NAME IS NOT ON THEIR LIST, NO PARTICIPATION OR PHYSICAL ACTIVITY OF ANY KIND WILL BE ALLOWED!

No paperwork or fees should ever be given to a coach.  The Athletic Office is open daily 7am-3pm.

1) Use link to complete online portion

2) Valid physical on file with athletic office (upload from link above or hard copy if submitting a new physical for 2018-2019).

3) Pay the athletic fee through RevTrak

Step #1

A physical must be on file for all student-athletes.  Most students complete the physical form during their freshmen and junior years.  Please reference the table below for valid dates in regard to all physicals.  Our new online system allows families to either upload their physical during the registration process or turn in their hard copy when ready.

Physical Date Valid School Year(s)
After 4/1/17 2018-2019
After 4/1/18 2018-2019 & 2019-2020

Step #2

Student-athletes must have a minimum 2.0 GPA with no more than one F/I.  When quarter and semester grades are available at a grade checkpoint, the more favorable grades for the student will be used to determine eligibility.  Incoming freshmen for the 2017-2018 school year will not have their grades officially checked until the end of 1st quarter.

Step #3

Student-athletes must pay a $150 athletic fee in order to participate in athletics at any level.  There is no sport limit, and no family cap.  If the student-athlete does not make the team, or chooses to leave the team prior to the first competition date, a full refund may be requested.  All fees are to be paid through Rev Trak.

Step #4

Student-athletes should have no outstanding financial obligations owed to the school district at the time of registration.

Step #5

All incoming freshmen, new/transfer students, as well as any student-athlete who did not participate in athletics at Waukesha West High School during the previous school year must attend our Freshmen/New Student Orientation Meeting, typically held in August.  The date of 2018's meeting is Tuesday, August 14th, 6:15pm. During this meeting we'll discuss the Code of Conduct, WIAA Eligibility, and many other relevant topics.

Step #6

Student-athletes and parent(s)/guardian(s) must complete the state-mandated Concussion Sign-off.  This is included within the online registration.

Step #7

Student-athletes and parent(s)/guardian(s) must complete the School District of Waukesha Spectator Code of Conduct.  This is included within the online registration.

Step #8

Student-athletes and parent(s)/guardian(s) must complete a Health Information Form and make any necessary updates each sports season within Infinite Campus.  This is included within the online registration.
